Care ManagerHoola Health | Bengaluru | Full-time | Work From Office

About Hoola HealthParenting is filled with questions, challenges, joyful moments, and sleepless nights. Through it all, every parent deserves a support system that understands, guides, and reassures them.

Hoola Health, formerly BabyMD, was founded by Deeksha Senguttuvan, whose experience in healthcare and technology highlighted the gaps families face while navigating paediatric care. In 2024, Hoola Health began with a simple mission: to make paediatric healthcare closer, calmer, and kinder.

Today, Hoola Health is a growing network of neighbourhood clinics offering paediatric consultations, vaccinations, developmental assessments, and therapy services. We are building a more accessible, personalised, and parent-friendly healthcare experience where families feel supported at every stage of their child's development.

About the RoleWe are looking for a compassionate, organised, and proactive Care Manager to be the trusted point of contact for families enrolled in our care programs.

In this role, you will guide parents through their child's healthcare journey, address common concerns using established clinical guidelines, coordinate consultations with specialists, and ensure timely follow-ups. You will work closely with paediatricians, nutritionists, lactation consultants, sleep consultants, and child-development experts to deliver a seamless and personalised care experience.

The ideal candidate combines healthcare knowledge with strong communication skills, emotional intelligence, and the ability to remain calm under pressure. Your goal is to ensure that every family feels heard, informed, reassured, and confident throughout their journey with Hoola Health.

Key Responsibilities1. Family Onboarding and Relationship Management

- Contact every new family on the day of enrolment and provide a clear introduction to the program, services, and available support.
- Understand each family's needs, expectations, and concerns to establish a supportive relationship from the beginning.
- Serve as the primary point of contact for parents throughout their care journey.

2. Parent Support and Guidance

- Respond to parent queries through phone calls and WhatsApp with empathy, professionalism, and timely communication.
- Address common concerns related to infant feeding, crying, sleep routines, and general childcare guidance using approved training materials and protocols.

- Help parents understand available resources and navigate the care program confidently.
- Maintain a calm, reassuring approach when interacting with anxious or distressed parents.

3. Clinical Coordination and Escalation

- Coordinate appointments and follow-ups with paediatricians, nutritionists, lactation consultants, sleep consultants, and child-development specialists.
- Identify concerns that require clinical assessment and escalate them promptly to the appropriate healthcare professional.
- Follow established escalation protocols for urgent concerns and ensure appropriate handover to the clinical team.
- Work within the defined scope of the role without independently diagnosing conditions, prescribing treatment, or replacing professional medical advice.

4. Personalised Care Planning

- Gather relevant information about each child's development, dietary habits, family needs, and care goals.
- Collaborate with relevant experts to support personalised nutrition, activity, and developmental plans.
- Communicate expert recommendations clearly to parents and support them in understanding the next steps.

5. Family Engagement and Follow-ups

- Conduct weekly check-ins with enrolled families to understand progress, address concerns, and encourage engagement with available services.
- Track participation in consultations, sessions, and educational resources.
- Identify families requiring additional support and coordinate appropriate interventions.
- Conduct monthly developmental reviews by collecting milestone and dietary information, coordinating report preparation, and explaining findings in collaboration with qualified experts.

6. Documentation and Operations

- Maintain accurate and up-to-date family records, interaction notes, appointment details, feedback, and care-plan changes.
- Track follow-ups, pending actions, escalations, and resolutions to ensure continuity of care.
- Use digital tools, spreadsheets, and internal systems to manage family interactions and monitor engagement.
- Maintain confidentiality and handle family and child health information responsibly.

7. Continuous Improvement

- Collect parent feedback and identify recurring concerns or service gaps.
- Collaborate with clinical and operations teams to improve care delivery and the overall family experience.
- Contribute ideas to improve parent engagement, communication workflows, and program effectiveness.

Eligibility CriteriaEducational Qualification — Mandatory
