About the Role
Cordia Healthcare Private Limited is seeking a motivated, empathetic, and patient-focused Health Coach to support patients in managing their health and improving their lifestyle through regular guidance, follow-ups, and digital health monitoring. The role involves reviewing patient progress through Cordia's healthcare dashboard and app, providing practical guidance on healthy lifestyle habits, encouraging adherence to care plans, and supporting patients throughout their wellness journey.

Key Responsibilities

- Review patient data and progress through the healthcare dashboard and app.
- Make regular follow-up calls to patients and provide guidance on diet, physical activity, exercise, and healthy lifestyle habits.
- Help patients understand and use the app, including daily health tracking and progress updates.
- Monitor patient engagement, identify gaps in adherence, and encourage patients to stay consistent with their care plans.




- Maintain call records, document patient progress, and coordinate with doctors or the internal team whenever needed.
- Support patients throughout their wellness journey with regular motivation and practical guidance. Preferred Profile
- Candidates with a background in Nutrition, Dietetics, Life Sciences, Physiotherapy, Psychology, or other healthcare-related fields are preferred.
- Valuable communication skills and an empathetic approach to patient care are essential. Application Question(s):
