Location:
Tezpur, Assam (On-site, Full time) Eligibility:
• Graduate in any discipline. Qualification in Medical Records Technology, Health Information Management or Hospital Administration will be preferred.
• Minimum 1+ years of experience in medical records management, preferably in a multi-speciality hospital.
• Working knowledge of medical coding (ICD-10) is essential.
• Knowledge of medical terminology, clinical documentation and patient records management.
• Familiarity with HIS/Hospital ERP and MS Office.
• Strong organisational skills, attention to detail and data accuracy.
• Understanding of patient confidentiality and medical records management protocols.

Key Responsibilities
• Manage and maintain patient medical records, ensuring accuracy, completeness and proper documentation.
• Review patient files for completeness of clinical notes,



investigation reports, discharge summaries and other relevant documents.
• Perform medical coding using applicable ICD-10 classification standards.
• Ensure accuracy and consistency of diagnosis coding and coordinate with clinical teams for documentation clarification, wherever required.
• Manage collection, indexing, filing, retrieval and archiving of medical records.
• Monitor discharge documentation and coordinate with clinical departments for pending records.
• Maintain OPD, IPD, emergency and other patient records.
• Ensure confidentiality, security and controlled access to patient medical information.
• Generate medical records-related reports and maintain necessary registers.
• Support internal audits, NABH documentation and compliance requirements related to medical records.
• Ensure adherence to hospital policies and applicable statutory requirements for medical record retention and management.
