1. Pre-Dialysis Preparation
-
- Check and record patient pre-dialysis weight, blood pressure, temperature, and pulse.
-
- Inspect and assess vascular access sites (AV fistula, grafts, or central venous catheters) for patency and signs of infection.
- Prime, calibrate, and set up the hemodialysis machine, blood tubing lines, and dialyzer according to doctor prescriptions and safety checks. 2. During Dialysis Treatment
-
- Cannulate vascular access or connect the patient safely to the dialysis machine.
-
- Monitor the patient's vitals, machine parameters, and ultrafiltration rates continuously throughout the session.
-
- Administer prescribed intra-dialysis medications, heparin, or saline as directed by the nephrologist.
- Identify and rapidly manage intra-dialysis complications such as drops in blood pressure (hypotension), nausea, vomiting, or muscle cramps. 3.
Post-Dialysis & Patient Care
-
- Safely disconnect the patient, apply proper pressure/dressings to access sites, and record post-dialysis weight and vitals.
-
- Clean, disinfect, and maintain dialysis equipment according to hospital infection-control guidelines.
-
- Maintain accurate electronic or physical nursing documentation of treatment sheets, fluid removal, and lab results. 4. Patient Education & Counseling
-
- Educate patients and their families on fluid and dietary restrictions (e.g., sodium, potassium, and phosphorus management).
-
- Instruct patients on how to care for their vascular access at home and recognize early signs of infection or clotting.
