We are looking for a highly organised and people-focused Service Coordinator to join our small, committed Community Living team. Working under the ACC Maximise Independence contract, you will play a key role in supporting kiritaki to live safely, confidently, and independently in their own homes. This is primarily a coordination and service delivery role, where you will ensure services run smoothly day-to-day.
You will work closely with the Service Manager and Clinical Coordinator (Registered Nurse) to coordinate supports that align with each client's needs and ACC-approved plans.As a Registered Health Professional, you will also bring valuable clinical insight and have the opportunity to step into broader leadership and service management support when required, contributing to the overall resilience and capability of the team.
Key responsibilities include:
- Coordinating the day-to-day delivery of home and community support services
- Developing and maintaining staff rosters to ensure consistent, reliable care when required
- Matching Support Workers to clients based on skills, preferences, and needs
- Responding to roster changes, staff availability, and urgent cover requirements
- Acting as the first point of contact for clients, whanau, and support workers (non-clinical matters)
- Supporting onboarding and coordination of new clients
- Maintaining accurate service records, timesheets, and documentation
- Identifying issues early and escalating clinical concerns appropriately
- Supporting the Service Manager with operational oversight as required
- Providing additional clinical or professional input when needed within scope

Ko wai koe - About you
You will:
- Be a Registered Health Professional (e.g. Nurse, Physiotherapist, Occupational Therapist, Social Worker)
- Have a minimum of three years post qualification
- Have experience in coordination, rostering, or service delivery roles (health, disability, or community sector preferred)
- Have proficient computer literacy and administrative capability
- Be highly organised, with strong attention to detail
- Have excellent communication and relationship-building skills
- Be calm under pressure and demonstrate emotional intelligence
- Be comfortable working independently in a small team
Ideally, you will also have:
- Experience in community-based health, disability, or rehabilitation services
- Knowledge of ACC or similar funding environments
- Experience working with people with complex needs (e.g. brain injury, spinal injury)
