Rate: $31.30 per hour + a premium of $3.00 per hour

Vacation: 4 weeks to start

The Clinical Counsellor is a regulated professional and member of Fred Victor's Health Services team, providing psychotherapeutic services to clients of Downtown East Toronto HART Hub. The position provides accessible, trauma-

informed, client-centred clinical counselling and psychotherapy to individuals experiencing homelessness, substance use, mental health challenges and other complex barriers to health and housing stability.

The Clinical Counsellor conducts clinical assessments, develops and implements individualized care plans, provides psychotherapy and psychoeducation, supports crisis intervention and safety planning, and works collaboratively with an interdisciplinary team to coordinate services and promote client wellness, recovery and stabilization. The position provides services in a range of settings, including community and partner locations, and works collaboratively with HART

Hub and Fred Victor staff and external service providers.

The Clinical Counsellor practices within the scope of their professional registration and applicable legislation, regulatory standards, Fred Victor policies and procedures, and the HART Hub model of integrated care. The position receives clinical consultation and supervision as appropriate and contributes to the ongoing development and evaluation of clinical services.

What You Will Do:

Clinical Assessment and Counselling

- Conduct comprehensive biopsychosocial assessments and identify client strengths, needs, risks and goals.
- Develop,



implement and regularly review individualized clinical and care plans in collaboration with clients and

the interdisciplinary team.
- Provide individual and/or group psychotherapy and counselling within the scope of professional practice.
- Apply evidence-informed, trauma-informed, and recovery-oriented approaches appropriate to the client's

needs.
- Provide psychoeducation related to mental health, substance use, trauma, coping, emotional regulation and

recovery.
- Exercise independent clinical judgment in assessing complex and changing mental health, substance use and

psychosocial presentations and determining appropriate clinical interventions, referrals and escalation of care. Crisis Intervention and Risk Management
- Assess and respond to changes in client mental health, substance use, safety and risk.
- Develop safety and crisis plans collaboratively with clients and the care team.
- Respond to urgent clinical situations and make referrals or escalate concerns in accordance with established

procedures.
- Consult with appropriate clinical and community partners when additional intervention or support is required.

Care Coordination and Collaboration
- Participate in interdisciplinary case consultation, care planning and case reviews.
- Coordinate referrals and link clients with appropriate health, mental health, substance use, housing and

community services.
- Work collaboratively with CTI workers, outreach workers, primary care providers, peer workers and partner

agencies to support continuity of care.
- Advocate for client access to appropriate services and supports.

Documentation and Qualified Practice
- Maintain accurate, timely and comprehensive clinical records in accordance with professional standards,

legislation, privacy requirements and Fred Victor policies.
- Maintain service statistics and complete program documentation and reporting requirements.
- Practice within the scope of professional registration and applicable legislation and regulatory standards.
- Participate in clinical supervision, consultation and professional development.
- Maintain professional knowledge and competence in accordance with the requirements of the individual's

regulatory college. Outreach and Community-Based Service
- Provide counselling and clinical services in office, partner and community settings as required.
- Adapt service delivery to reduce barriers to accessing mental health services, including flexible and outreach-

based approaches.
- Maintain awareness of personal and client safety when providing services in community settings.

Team and Program Responsibilities
- Participate actively in team meetings, case conferences and organizational initiatives.




- Contribute clinical knowledge and expertise to interdisciplinary team learning and service development.
- Participate in program evaluation, quality improvement and development of clinical practices.
- Promote Fred Victor's mission, vision, values and commitment to anti-oppressive, culturally responsive and

client-centred practice. What You Bring to the Team:

- Master of Social Work (M.S.W.) or a graduate degree in a relevant field, licensed to practice under the

Regulated Health Professions Act.
- Registration and good standing with a recognized regulatory body (e.g., Ontario College of Social Workers,

College of Registered Psychotherapists of Ontario, or equivalent).
- Minimum of 3 years of related experience in mental health, addictions, and/or social services performing

clinical counselling and assessments.

oppressive, anti-racist, trauma-informed, harm reduction, and mental health recovery frameworks.
- Proven experience in clinical counselling and direct client service delivery.

Compensation and Benefits:

- For the Clinical Counsellor, Fred Victor offers an hourly rate of $31.30 + a $3.00 premium per hour
- 4 weeks of vacation to start + 1 float day
