Care Navigator – Social Worker
Location: Remote – Cook County, Illinois
Travel: Local travel to DCFS offices approximately twice per week

We are seeking an experienced Social Worker/Care Navigator to assess member needs, coordinate healthcare and social services, develop care plans, and connect members and families with appropriate community resources.

Responsibilities:

- Assess needs and barriers to care and develop care plans.
- Coordinate services with members, families, providers, and community organizations.
- Connect members with resources such as housing, food, employment, education, and social services.
- Provide advocacy, education, and psychosocial support.
- Track care-plan progress and maintain accurate documentation.
- Conduct community/onsite visits as needed.

Requirements:

- Bachelor’s degree in Social Work, Nursing, Behavioral Science, or related field.
- Approximately 6 years of related social work/care coordination experience.
- Strong communication, advocacy, case management, and documentation skills.




- Experience with healthcare, community resources, child welfare, or foster care is preferred.
- Current Illinois clinical/social-work license preferred.
- Ability to travel locally within Cook County twice weekly.
