Key Responsibilities
Assist physicians in patient assessment and clinical documentation.
Review patient medical histories, medication profiles, laboratory reports, and treatment plans.
Support medication reconciliation and identify potential drug interactions, duplications, allergies, and medication-related problems.
Provide appropriate medication-related information to physicians and the clinical team.
Monitor patients for medication-related adverse effects and therapeutic outcomes.
Assist in preparing and maintaining patient case sheets and clinical records.
Coordinate with doctors, nurses, pharmacists, laboratories, and other healthcare specialists.
Participate in ward rounds and clinical discussions as required.
Follow up on investigations, reports, and physician instructions.
Educate patients regarding medications, adherence, precautions, and lifestyle-related aspects as directed by the treating physician.
Ensure accurate and timely clinical documentation.




Maintain patient confidentiality and comply with hospital policies and SOPs.
Support quality improvement, clinical audits, and patient-safety initiatives.

Eligibility
Pharm.D / Pharm.D (PB) from a recognized institution.
Candidates with internship/clinical posting experience are preferred.
Freshers are welcome to apply.
Positive understanding of pharmacology, therapeutics, clinical pharmacy, and patient care.
Good communication and interpersonal skills.
Willingness to work in a hospital/clinical setting and rotational shifts if required.
